more obvious way to set "parent page" for wiki page

Description
Did you know that pages in the wikis here can have parents? And if a page has a parent, it gets displayed with a breadcrumb path at the top.
Unfortunately the parent doesn't get set by default and there is no way to set it in the form for creating the page in the first place. You have to click to "rename page" after creation and then set the parent in there. As a consequence I was totally unaware that this feature existed (Steve W just asked me specifically if it was possible to do this and I said I thought it wasn't).
It'd be jolly nice if this were more obvious.

What's more, setting up a page hierarchy in this way actually makes the "Index by Title" link on the right side of every wiki page rather useful -- see for example http://code.soundsoftware.ac.uk/projects/sodamat/wiki/index
