Miscellaneous small refactorings:

* conditionalise signals code on O2_DEBUG;
* move audioDB initializers to .h file;
* reindent code according to One True Coding Standard;
* query types live in a different namespace to DB flags; preserve all
  values (for binary compatibility) but make that clearer in the header
  file.
